Primary Care Networks (PCNs) are a key part of the NHS Long Term Plan. A PCN consists of a group of GP practices working together, to improve and build upon core of primary care services, to enable a greater provision of care and to meet the changing needs of the community.
PCNs aim to deliver improved levels of care for patients, by connecting the primary healthcare team across the area with local community service providers, serving to alleviate workload and share staff. This allows a new, more streamlined, multi-disciplined, personalised and coordinated approach to patient care. The system supports patients to take greater control over managing ongoing conditions, introducing new roles to increase services provided and highlighting alternative care providers in the area.
With this emergence comes a change in the way primary care professionals work; typically, a PCN role will be based across multiple practices, affording more freedom and flexibility for the workforce and enabling better delivery of care across the area.
