A Health and Wellbeing Coach takes a holistic approach; to support, educate and motivate patients to take a more active role in their own health and physical wellbeing, and make positive and confident behavioural changes. The role of the Health and Wellbeing Coach will be broad and varied and will include areas such as healthy eating and physical activity, in order to help patients to achieve desired health objectives. This may include assisting patients with goal setting and working towards them, working with them to a create personalised care plan, developing bespoke programmes, all with the aim of increasing overall health, preventing further illness and slowing deterioration of existing conditions where possible.
The role of the Health and Wellbeing Coach is to encourage and coach patients to become more independent, resilient and engaged with their own health and wellbeing – and to set, and work towards their own health objectives.
This role may involve:
- Working in partnership with a multi-disciplinary team to support the PCN’s overall strategy and objectives
- Working closely with a wide range of patients to develop bespoke health and wellbeing plans
- Educating patients on how to look after their own health and wellbeing more effectively
- Using coaching skills to encourage and promote confidence and motivation for patients to work on their own self-care and to access community support

- Empathetic, with great organisational skills

- Able to work autonomously
- Able to manage a busy and varied workload
Entry requirements and skills:
- Educated to GCSE or equivalent level
- Holding, or working towards, an NVQ level 3 or other relevant first level professional qualification may be advantageous
- Experience of working in primary care (and clinical systems) may be advantageous but not essential
- Excellent communication skills
- Strong IT skills, ideally including experience in data collection and report writing
